Applications for the 2025 grant cycle open on March 1, 2025, and must be submitted by May 31, 2025. The program is limited to residents of Illinois or Indiana who are currently receiving treatment for breast cancer and facing financial strain due to the cost of care. Only one award is permitted per household each year, with a maximum award value of $250. This assistance is intended to be one-time support per application cycle.

The grant application includes detailed questions about the applicant's diagnosis, financial need, daily challenges, and requested support type. Required documentation includes verification of medical treatment and financial need. Applicants are asked to provide an honest and thorough account of their situation, including how breast cancer has affected their employment, income, and access to resources. Each submission will be reviewed by the Pink Warriors Foundation Assistance Review Committee, with responses issued via email or mail within 2–3 weeks of submission.

As part of the application, individuals are asked whether they consent to sharing their story and photograph on the Pink Warriors Foundation’s website or social media platforms, though this is optional and not a condition for funding. Only complete and signed applications will be considered. By submitting the application, applicants authorize the Foundation to verify all provided information and acknowledge that all rules must be followed for consideration.
