Description
The CommonSpirit Health Equity & Advancement Fund was launched in 2021 to support Diversity, Equity, and Inclusion (DEI) initiatives in Colorado, Utah, and Western Kansas. This program aims to improve health outcomes and advance social justice for underrepresented and underserved communities by addressing Social Determinants of Health (SDOH), integrating community participatory approaches, and building community capacity with a focus on community health workers.
The fund prioritizes projects that align with the Mountain Region Social Justice Framework, collaborate with other organizations, fill unique health equity gaps, support successful care models, introduce innovative approaches to reducing health disparities, and leverage additional funding. Eligible applicants include 501(c)(3) nonprofits, community-based organizations with fiscal sponsors, and federally recognized tribal governments. The grant application period is from July 15, 2024, to September 9, 2024, with awards ranging from $10,000 to $50,000.
Eligible Geographies are within 50 miles of a CommonSpirit facility or a CommonSpirit-affiliated facility
