Description
The Small Grants under the Community Health Improvement Grants program are designed to support the health and well-being of vulnerable and underserved populations within communities served by CommonSpirit Health hospitals. These grants focus on enhancing community health and health equity through defined projects carried out by 501(c)3 non-profit organizations. The grants fund initiatives identified in the hospitals’ community health needs assessments, with projects expected to span 9 to 12 months. The application period for the 2025 grants is from June 17 to August 16, 2024, with funding decisions notified by December 18, 2024. The grant project period starts on March 1, 2025, and ends on February 28, 2026.
Hospitals in CommonSpirit’s Mountain Region do not participate in this program, focusing instead on a separate Health Equity & Advancement Fund.
Collaboration with partners is encouraged but not mandatory. Less detailed project information is required compared to standard grants.
