Description
The Standard Grants under the Community Health Improvement Grants program aim to enhance the health and well-being of vulnerable and underserved populations within communities served by CommonSpirit Health hospitals. These grants focus on improving community health and health equity through defined projects carried out by 501(c)3 non-profit organizations. The grants support initiatives identified in the hospitals’ community health needs assessments, with projects expected to span 9 to 12 months. The application period for the 2025 grants is from June 17 to August 16, 2024, with funding decisions notified by December 18, 2024. The grant project period starts on March 1, 2025, and ends on February 28, 2026.
Hospitals in CommonSpirit’s Mountain Region do not participate in this program, focusing instead on a separate Health Equity & Advancement Fund.
Proposals must include two or more collaborating partner organizations with distinct, complementary, and substantive project roles. Requires more detailed project information, including how funds will be distributed among collaborating partners.
This grant is more suitable for larger, more comprehensive projects that require significant funding.
