Community Health Improvement Workgroup Mini-Grant
This funding opportunity provides financial support for community health projects in Monroe County that address local health needs and social determinants of health.
Description
The Community Health Improvement Workgroup (CHIW) Mini-Grant offers funding to support community health initiatives in Monroe County for the year 2025. The program aims to fund one or more projects, either new or ongoing, with a total grant pool of $20,000. The grant period is for one year, and applications are due by November 15, 2024. Award notifications will be made in December 2024, allowing recipients to start their projects in the upcoming year.
Eligible projects should align with the priorities of the 2022-2024 Community Health Improvement Plan or address social determinants of health within the local community. The CHIW encourages proposals for initiatives that can have an impact on either a local, city-wide, or county-wide scale. Selected projects will receive support for health-related efforts that contribute to the overall well-being of Monroe County residents.
Grantees will be required to provide periodic updates on their project outcomes, but the reporting process is designed to be minimally burdensome. This includes submitting brief written progress reports and participating in quarterly CHIW meetings to share successes and developments.
This funding opportunity is ideal for organizations looking to implement or expand health initiatives that address identified community health needs and improve social conditions affecting health outcomes.
