Utah Primary Care Grant Program
This program provides funding to organizations that deliver primary healthcare services to low-income and uninsured individuals in Utah, aiming to improve access and health outcomes for underserved populations.
The Utah Primary Care Grant Program (UPCGP), administered by the Utah Department of Health and Human Services Primary Care and Rural Health (PCRH) office, is designed to provide funding to private entities, including both for-profit and non-profit organizations, to deliver primary healthcare services to medically underserved populations in Utah. The program aims to enhance access to ambulatory primary care services for individuals and families who earn low wages and lack health insurance, and who are not eligible for CHIP or Medicaid. It also covers primary care services not covered by Medicare, Medicaid, CHIP, or private insurance.
The UPCGP's objectives are to improve the health of eligible individuals, ensure continuity of care for those receiving ambulatory primary care services, increase the number of eligible encounters by individuals within the target populations, and enhance the organizational capacity of grantees to provide ongoing and long-term ambulatory primary healthcare services. Eligible applicants include private non-profit and public organizations that provide primary health care to low-income, uninsured, and under-insured individuals, with the goal of increasing their capacity to serve these populations.
The populations served by the UPCGP include workers earning low wages, children, the elderly, Native Americans, migrant and seasonal farm workers, individuals with chronic diseases, those experiencing homelessness, individuals with limited English-speaking proficiency, and single heads of household. Grantees are required to submit quarterly progress reports to demonstrate the impact of their services on these communities. These reports must include information on the volume of patients served, the quality of care provided, patient outcomes, and how the program has helped agencies achieve their goals.
The application cycle for the UPCGP opens on Monday, March 17, 2025, and closes on Monday, April 14, 2025. During this period, eligible organizations can submit their applications to receive funding. The program encourages agencies to include anecdotal stories in their progress reports to illustrate the program's success to funders and legislators.
