Description
The Blue Cross Blue Shield of Michigan Foundation is offering the Community Health Matching Grant Program to support health professionals and community leaders from nonprofit organizations in developing and implementing population-led, community-inspired programs. These programs are aimed at addressing community-identified needs and evaluating measurable programmatic and health outcomes. The grant provides up to $25,000 per year for two years, targeting nonprofit community organizations based in Michigan. Eligibility for the grant requires matching funds determined by the organization’s budget size and the percentage of Medicaid and uninsured patients served. The application process includes submitting a concept paper, receiving feedback, and potentially submitting a full proposal if invited.
