This funding opportunity supports nonprofit organizations in western and central Pennsylvania and West Virginia that are implementing innovative health initiatives to improve community and family health outcomes.
Description
The Highmark Foundation focuses on enhancing community and family health in western and central Pennsylvania, as well as West Virginia, by supporting initiatives within three broad categories: Chronic Disease, Family Health, and Service Delivery Systems. Funding priorities for 2020 are allocated as follows: Service Delivery Systems (63%), Family Health (32%), and Chronic Disease (5%). The Foundation supports evidence-based demonstrations, pilot projects, model programs, expansion plans, and well-established initiatives that aim to improve health outcomes and address public health issues. Projects eligible for support include those that demonstrate potential for long-term impact, address multiple counties, and attract collaborative funding. The Foundation’s grantmaking is proactive and research-driven, focusing on nonprofit organizations that offer innovative solutions to health challenges.
The Foundation’s board of directors meets three times per year (spring, summer, and fall) to review and approve proposals. Once a grant is approved, additional information, including a site visit, may be required. Applicants are notified by email once the proposal has been approved for funding.
