The U.S. Agency for International Development (USAID) has announced a funding opportunity under the Harmonization of systems and services to embed HIV service delivery into primary health care (HARMONIZE) program. The initiative aims to integrate HIV services into primary health care (PHC) systems, ensuring sustained epidemic control, strengthening health system resilience, and improving global health security. The program builds on past PEPFAR efforts by embedding HIV services within existing health structures to support a more holistic, person-centered approach.

USAID anticipates awarding one cooperative agreement under this funding opportunity, with an estimated $3 million in seed funding per year and additional field support buy-ins expected to reach a total of $780 million over five years. The estimated period of performance is five years, beginning around July 17, 2025. Eligibility for this award is open to all interested organizations, including local entities, nonprofit and for-profit organizations, universities, and faith-based groups.

The program’s three primary objectives focus on embedding HIV services within PHC facilities and community sites, strengthening health governance and system capacity, and improving investments in surveillance and response mechanisms to prevent emerging health threats. Activities under HARMONIZE will include supporting community-based HIV prevention and care programs, improving coordination between facility-based and community health services, strengthening data systems for integrated health monitoring, and training health care providers to offer comprehensive services.

Applications for this opportunity will follow a two-step process, beginning with the submission of a concept note by February 28, 2025. Selected applicants will be invited to submit a full application, with details on that deadline to be determined. Concept notes should outline the proposed approach to integrating HIV and PHC services, describe strategies for sustainability and capacity building, and demonstrate the applicant’s expertise in implementing large-scale public health programs. Submissions must be emailed to USAID by the deadline.

Evaluation criteria for concept notes include technical feasibility, innovation in service delivery, relevance of proposed monitoring and evaluation plans, and the applicant’s experience and organizational capacity. Full applications will require a more detailed technical approach, a management plan, staffing details, and a comprehensive budget.

USAID emphasizes collaboration with national and local governments, the private sector, civil society, and global health organizations. The agency encourages applications from organizations that have not previously received USAID funding and prioritizes initiatives that enhance local leadership and long-term sustainability.
