The Austin-Bailey Health and Wellness Foundation is committed to supporting programs that promote the physical and mental well-being of community members within its designated service area. This Foundation focuses its efforts on initiatives that directly impact the health and wellness of individuals residing in Holmes, Stark, Tuscarawas, and Wayne counties in Ohio. Its goal is to make meaningful contributions toward improving both physical and mental health in these communities.

Organizations seeking funding must serve all or part of the four-county region mentioned. Eligibility is restricted to organizations that are classified as tax-exempt under IRS regulations, specifically under the 501(c)(3) designation. Ineligible requests include annual appeals, membership drives, non-specified capital campaigns, fundraising events, program advertising, lobbying activities, and endowment funds. Proposals not following the provided guidelines will also be deemed ineligible.

To apply, organizations must first contact the Foundation office by phone to discuss their project and request application materials. Following the discussion, specific instructions for application preparation will be sent by mail or email. The application process begins with completing a Grant Request Cover Sheet, which helps determine alignment with the Foundation’s mission. Required submission materials include a description of the organization, its mission statement, project objectives, the current year’s budget, an IRS tax determination letter, prior year's audited financial statements, and a list of the board of directors.

Grants are awarded twice annually, with application deadlines set for May 27, 2025, and November 18, 2025. Applications must be received by 4:30 p.m. on these dates. Applications received by the May deadline will be reviewed and decided upon by mid-September, while applications received by the November deadline will be reviewed and decided upon in March of the following year. Applicants will be notified shortly after the board’s decisions. Submissions can be made at any point during the calendar year, but they will be queued for the next respective review period based on the date of receipt.
