Description
The Community Health Matching Grant Program, offered by the Blue Cross Blue Shield of Michigan Foundation, supports nonprofit organizations in Michigan in addressing community-identified health needs. The program emphasizes the development of population-led, community-inspired programs or implementation of evidence-based programs to improve health outcomes. Grants provide up to $25,000 per year for two years and require matching funds, determined by the applicant organization’s budget size and the percentage of Medicaid or uninsured patients served. Matches must come from an external source; applicants cannot serve as their own match.
The application process begins with the submission of a concept paper, which is reviewed by a Senior Program Officer to provide feedback and guidance. Organizations invited to apply must submit a comprehensive proposal, including a narrative, budget, sustainability plan, and other required materials. Applications are reviewed during one of three annual board meetings, with decisions provided within 10 business days. Deadlines for concept papers are at least 60 days before the target grant application deadline.
Eligible organizations must be Michigan-based nonprofits. Restrictions include disallowing funding for indirect costs, capital expenses, computer equipment, and salaries or tuition for project personnel. The grant prioritizes initiatives that address health equity, social determinants of health, and demonstrate measurable health impacts.
