Description

The Access to Health Care Community Grant for 2025, administered by the Oklahoma City Community Foundation (OCCF), supports nonprofit organizations that provide essential health care services to underserved individuals in Oklahoma, Cleveland, and Canadian Counties. This grant is aimed at organizations offering primary, diagnostic, preventative services, and hospice care to those who would otherwise lack access to these services.

Key Dates:
Information Meeting: October 29, 2024, at 3:00 p.m.
Letters of Interest (LOI) Deadline: December 5, 2024, by 5:00 p.m.
Grant Application Deadline: December 12, 2024, by 5:00 p.m.
Funding Decision Notification: End of February 2025.
Funding Range:
Grants: Between $5,000 and $40,000.
Eligible Projects: New or existing projects that provide direct health care services in the community.
Project Priorities:
Health Care Services: Projects providing medical, dental, mental health, or prescription drug services.
Early Detection: Services for the early detection or prevention of chronic conditions.
Parkinson’s Disease Services: Therapeutic services for patients with Parkinson’s or severe arthritis.
Hospice Care: Palliative care or social hospice services for individuals without access.
Eligibility:
Must be a 501(c)(3) nonprofit with a governing board located in central Oklahoma.
Must provide direct health care services in the defined counties.
Organizations may submit one application per grant cycle.
Ineligible Expenses:
Food distribution, transportation, capital campaigns, fundraising events, vehicle purchases, clinical equipment, or facility renovations. Administrative expenses may be considered but cannot dominate the budget request.
Application Process:
Step 1: Attend the optional Information Meeting.
Step 2: Submit an LOI through the OCCF online grant portal by December 5, 2024.
Step 3: If invited, submit a full grant application by December 12, 2024.
Step 4: Applicants will be notified of the funding decision by the end of February 2025.
