The Department of Health and Human Services (HHS), through the Administration for Community Living (ACL), has forecasted a discretionary grant opportunity aimed at reducing falls and associated risks among older adults and adults with disabilities. The initiative will support the development and implementation of evidence-based falls prevention programs. The primary goal is to empower these populations to mitigate the risks of falls through education, access to effective programs, and strategic outreach.
This opportunity will be administered via cooperative agreements, ensuring close collaboration between the ACL and awarded organizations. It is designed to facilitate trauma-informed and person-centered approaches tailored to the needs of individuals with behavioral health issues and/or chronic conditions. The strategy will include developing strong partnerships with relevant organizations that serve targeted populations, and all deliverables and resources created must comply with Section 508 accessibility standards.
Funding will be awarded to four recipients, with each award ranging between $750,000 and $1,000,000. The total funding available for this initiative is $4,000,000, made available through Prevention and Public Health Funds under Assistance Listing 93.761.
Eligibility is broad and includes various government entities such as city, county, state, and special district governments, as well as public and private institutions of higher education, public housing authorities, Indian housing authorities, independent school districts, and both federally and non-federally recognized Native American tribal governments. Nonprofit organizations with 501(c)(3) status and faith-based or community organizations meeting the eligibility criteria are also encouraged to apply. Foreign entities, however, are not eligible.
The estimated timeline indicates that the funding opportunity will be posted by April 23, 2025. The application deadline is set for June 23, 2025, with awards expected to be announced and projects initiated by September 1, 2025. No cost-sharing or matching is required for this grant.
