Description
The Health & Wellness Community Grant aims to improve health outcomes among under-resourced populations by addressing barriers to affordable, quality health care, and encouraging healthy behaviors that reduce chronic disease prevalence. The program recognizes that factors such as education, environment, behavior, and genetics influence health, but that many communities face systemic challenges in accessing healthcare services.
The grant supports programs that enhance participant access to primary care, dental services, and mental/behavioral health services, including substance abuse treatment. It also funds sustained programs that encourage healthy behaviors—specifically those that prevent or reduce chronic diseases like Type II diabetes, heart disease, and lung disease. Preference is given to programs that incorporate health literacy initiatives, helping participants better understand and manage chronic health conditions.
Successful grant proposals should aim to achieve one or more of the following outcomes: connecting individuals with health disparities to a medical home, ensuring participants complete a dental treatment plan, improving behavioral health conditions, or increasing participants' knowledge and behaviors related to chronic disease prevention. Minor equipment needs may be included as part of the grant request. However, major capital projects must be submitted under a separate funding stream (Vibrant Places grants).
