Yamhill Community Care Organization (YCCO) has released its Spring 2025 Request for Proposals (RFP) to support innovative and impactful community-based projects aligned with its mission of improving the quality of life and health of residents across its service areas. The RFP is designed to fund projects that contribute to physical, mental, emotional, spiritual, and social well-being, in accordance with YCCO's vision and its Community Health Improvement Plan (CHIP). Applicants must submit proposals by April 11, 2025, at 11:59 PM PST through the YCCO online application portal.

Eligibility is limited to 501(c)(3) organizations, tribes, and public agencies. While multiple organizations may collaborate, only one agency may serve as the fiscal sponsor. All projects must serve residents in Yamhill, Polk, or Washington Counties in Oregon. Proposals must address at least one of the Social Determinants of Health and Equity (SDOH-E), demonstrate a clearly defined community need, include at least three measurable outcomes, and contain a sustainability plan. Projects must not include healthcare service delivery or supplant existing funding streams.

Applicants may choose from five distinct funding streams: Community Health Improvement Plan (CHIP) projects with priorities like housing and youth services ($5,000–$20,000), Traditional Health Worker (THW) support ($4,000–$15,000), Supporting Health for All through Reinvestment (SHARE) initiatives which can include capital projects ($60,000–$250,000), and Community Prevention and Wellness (CPW) programs focused on evidence-based, population-level prevention strategies (average $100,000). Projects addressing disparities and involving underrepresented populations in development and implementation will be more competitive.

A Technical Assistance webinar is available online, and YCCO staff will provide support from March 3 to April 11, 2025, during business hours. A Q&A opportunity is available during this time. Award notifications will be issued by June 18, 2025, and all grant contracts must be executed by July 31, 2025. A separate application, the Community Capacity Building Funding (CCBF), may also be pursued concurrently.

Evaluation criteria include alignment with YCCO’s mission and CHIP (25%), engagement of underserved populations (25%), expected impact (20%), sustainability (20%), and application quality (10%). Applications must be submitted through the designated portal and include contact information, project details, evaluation plans, outcome measures, timelines, a sustainability plan, a detailed budget, disclosures, and required documentation including insurance and background check compliance.
