Description
The Preventative Care Access grant offered by the Patterson Family Foundation focuses on expanding innovative healthcare delivery systems that increase access to preventative care in rural communities. This funding opportunity is geared toward organizations implementing or scaling services such as mobile integrated health, community paramedicine, community health workers, telehealth delivery, and mobile health units. These approaches are aimed at reducing healthcare access barriers and improving outcomes for rural populations through early intervention and sustained outreach.
Grants range from $100,000 to $500,000, making this program suitable for both pilot projects and more developed expansions. The Foundation welcomes proposals that demonstrate creativity, potential for scalability, and a clear impact on community health through preventative approaches. However, funding cannot be used for electronic medical records (EMRs), payroll or benefits, insurance, or general operating costs such as rent, utilities, or facility maintenance.
Eligibility is restricted to organizations operating in rural counties in Kansas and western Missouri with populations under 50,000 residents. The grant follows a two-round annual cycle. Round 1 opens on April 1, 2025 at 12:00 PM CST, with LOIs due by April 18, and full applications due by May 9. Round 2 LOIs are due by July 3, with full applications due by July 25, 2025. Funding determinations will be communicated in August (Round 1) and October (Round 2).
All applicants must register via the Foundation’s Grants Portal before submitting a Letter of Intent. Registration must be completed by 12:00 PM CST on April 18 for Round 1 and July 3 for Round 2. Eligibility is reviewed during the registration process, and only approved organizations will receive access to the LOI application. Returning users can log in directly with existing credentials.
The Foundation provides Office Hours from 1 – 3 PM, allowing organizations to receive personalized application support. The Patterson Family Foundation emphasizes a competitive and transparent review process.
