The 2025 Social Determinants of Health Grants program aims to address the critical factors that influence health outcomes, focusing on the conditions in which individuals are born, grow, live, work, and the systems that shape these conditions. Grants
will be awarded to initiatives addressing one or more of the following categories: economic stability, education access and quality, healthcare access and quality, neighborhoods and safe housing, and social and community context. Organizations may apply for grants ranging from $25,000 to $250,000. The process begins with a Letter of Intent (LOI), which opens on January 1 and closes on January 12, 2025. The application period will run until February 14, 2025, giving applicants nearly three weeks to provide detailed proposals.
