Description

The DC Department of Health (DC Health) is offering the Community Access Innovations (CAI) grant to support innovative strategies aimed at reducing racial and ethnic disparities in healthcare access in Washington, DC. This initiative focuses on piloting the development of community healthcare access hubs to prevent and reduce maternal morbidity and mortality. The grant is administered by DC Health's Family Health Bureau, within the Community Health Administration.

Eligible applicants include Federally Qualified Health Centers (FQHCs), FQHC Look-Alikes, nonprofit organizations (such as nonprofit primary care clinics and birthing facilities), and for-profit organizations that provide clinical services within a primary medical Health Professional Shortage Area. Individuals are not eligible for this funding opportunity. Applicants must have a DUNS number, a Tax ID, and be registered in the federal Systems for Award Management (SAM) with an active UEI number to apply through DC Health’s Enterprise Grants Management System.

The total anticipated funding available is $1,200,000, with annual awards ranging from a minimum of $200,000 to a maximum of $604,000. DC Health expects to issue up to two awards. There is no cost-sharing or matching requirement for this grant.

The Request for Applications (RFA) will be released on December 6, 2024. The application deadline is January 28, 2025, at 3:00 p.m. Applications must be submitted electronically via DC Health’s grants platform.
