The 2025 Maternal and Child Health Summit Provider Funding Application offers up to $10,000 for a one-year project aimed at planning, executing, and facilitating two virtual Maternal and Child Health Summits in 2025. These summits are for Title V grantees, including local and Tribal health agencies, and will address diverse maternal and child health topics. The project begins on January 1, 2025, and concludes on December 31, 2025.

The goal of this funding opportunity is to support the Wisconsin Department of Health Services (DHS) in promoting maternal and child health through strategic summits.
