The 2025 Maternal and Child Health Summit Provider Funding Application offers up to $10,000 for a one-year project aimed at planning, executing, and facilitating two virtual Maternal and Child Health Summits in 2025. These summits are for Title V grantees, including local and Tribal health agencies, and will address diverse maternal and child health topics. The project begins on January 1, 2025, and concludes on December 31, 2025.
The goal of this funding opportunity is to support the Wisconsin Department of Health Services (DHS) in promoting maternal and child health through strategic summits. Applicants are expected to align with the Title V program’s priorities, which include advancing equity and racial justice, ensuring access to quality health services, fostering supportive social connections, improving perinatal outcomes, promoting positive mental health, and encouraging optimal nutrition and physical activity.
Funding for this grant is contingent on resources made available to Wisconsin DHS by the Health Resources and Services Administration (HRSA) under the Title V Maternal and Child Health Grant. Selection is based on factors including the applicant's capacity to deliver on project goals, adherence to application criteria, and availability of funds.
Eligible applicants include organizations capable of coordinating statewide virtual events. The selected agency must demonstrate expertise in maternal and child health, event planning, and virtual facilitation, as well as an ability to address the priorities of Wisconsin’s Title V program.
Applications must be submitted via the funding application survey by December 30, 2024, at 5:00 pm Central Time. Late submissions will not be considered. The survey serves as the primary application mechanism, and applicants must meet all outlined criteria to be eligible.
